BUG: cellSizeFunction. Bad logic for triggering warning for bothSides

default whern surface does not support volumeType.
BUG: conformalVoronoiMesh. Making test "totalHits >= hitLimit" rather than
"totalHits = hitLimit" to be strictly consistent and always trigger
the iteration termination message.

BUG: globalMeshData::mergePoints was assuming patch was on master processor.
...
Each collocated set of coupled points has a single master assigned to it.
However when doing mergePoints on a patch the master does not have to be
on the patch. So now it returns mesh point labels, not patch point labels.

ENH: Distribution. Basing the Distribution class on a List instead of

a Map.
Resizing by doubling and mapping data when a new entry is added that
is outside of the current bounds. Trimming the output (normalised or
raw) to only the entries that contain data.
Overriding List clear method to enable the Distribution to be reset,
but retaining the correct number of components.

BUG: meshes with differing pointsInstance and facesInstance were not decomposed correctly.
...
In case of differing pointsInstance and facesInstance it will
- read the points belonging to the facesInstance
- construct and write the mesh belonging to the facesInstance
(so with the old, facesInstance, points)
- additionally write the current points to pointsInstance
